Individual healthcare plans

Individual healthcare plans (IHPs) help settings to effectively support children with medical conditions especially where these are long-term and complex.

They provide clarity about what needs to be done, when and by whom. They are of particular importance in cases where conditions fluctuate or where there is a high risk that emergency intervention will be needed.

Plans should be drawn up in partnership between the setting, parents, and a relevant healthcare professional such as the:

  • Health visitor

  • Dietician

  • Children’s community nurse or Paediatrician

Children should also be involved whenever appropriate. Where the child has a special educational need identified in an EHC plan, the individual healthcare plan should be part of that plan.

Diabetes

Buckinghamshire Council and Buckinghamshire Healthcare NHS Trust have published updated Good Practice Guidelines for Children and Young People with Type 1 Diabetes in Early Years, schools and education settings.

The guidance outlines:

  • statutory responsibilities

  • expectations for Individual Healthcare Plans

  • staff training requirements

  • exam arrangements

  • funding processes

  • the use of diabetes technology to support inclusion and wellbeing in educational settings

Settings are encouraged to review the guidance and ensure their policies and practices reflect current expectations
