Parent/Provider Agreements (PPA) for the Funded Entitlement

The Parent/Provider Agreement is a contract between the provider and the parent/carer for the accessing of the Funded Entitlement hours. The form is used to gather the child’s details from parents/carers and record the number of Funded Entitlement hours that are being accessed. This form is evidence that providers have permission from the parent/carer to be able to share information with Buckinghamshire Council, the Department for Work and Pensions, HMRC, and the Department for Education, if required.

The Parent/Provider Agreement needs to be completed in full, before the child’s first funded term and providers must hold signed copies for each child claiming the Funded Entitlement. These should be reviewed on an annual basis to ensure funding is being used correctly. If a child is attending more than one provider, a PPA must be completed by both providers, and both signed by parents/carers. The form must be updated and initialled by parents/carers when there is a change in the hours claimed or the child becomes eligible for a different funding stream.
