Understanding your claim form

Claim forms sent to you through the secure file transfer site by the EY Funding Team are in Microsoft Excel format. You will need to ensure that you can access and save as this format to successfully complete and return your claim forms.

Provisional and final claims follow the same format, with one or 2 additional fields only needing to be completed on the final claim each funding period.

This document is intended to familiarise you with the claim form and help you understand what is expected during completion.

Before you start to complete the claim form

Once you have downloaded and saved your claim form from AnyComms, you are ready to start completing your claim. If you need any help with how to download, please refer to the user guides here.

To start completing your claim form please ensure that you have the PPAs for all children you need to submit a claim for to hand and that these are up to date with any changes. Please also ensure, where required, that you have checked working parent eligibility codes for children on your portal and that you are happy that the codes meet the requirements for the term you are going to complete the claim for.

When you first open the claim form, you will be presented with the instruction screen, and you will also notice that there are several named tabs along the bottom of the screen. Please take some time to read the instructions before you start as these will help you avoid errors and help you understand what is expected with some of the areas of input.
